Congenital Heart disease Project in the north area of Cd. Juárez, Chihuahua

The present project raises the repercussion in the patients’ care with congenital heart disease in Cd. Juárez and the proposal of care in relation to real expenses and the need for a third level for the population from the city and the state of Chihuahua.

According to a medical study performed in Cd. Juárez that covers the period from 2014 to 2017, in public and private hospitals except for the IMSS. The total number of diagnosed patients with congenital heart disease were 178 cases (49.5% of the studies requested) with 50.5% of these confirmed cases being patients from the Hospital Infantil de Especialidades.

In this same period of time from 2014 to 2017, 35 cases of death were documented at the Hospital Infantil de Especialidades, which represent the 19.6% of the 178 diagnosed cases. During 2018, the mortality increased to 19 cases, a 211% more according to the death records compared to previous years. In regards to the Instituto Mexicano del Seguro Social, in the Hospital Regional No. 66, one of the three important hospitals of the IMSS in Cd. Juárez, a total of 213 patients with a diagnosis of congenital heart disease were documented over the course of two years, from January 2016 to December 2017, of which 80% (167n) are surgical and 21.5% (46n) are not-surgical. Currently, there are more than 200 cases waiting for surgery of adult and pediatric patients in the 3rd level that corresponds to the UMAE 71 in Torreón, Coahuila.

At this time, in the HGR No. 66 there is a list of 37 patients for surgery of which 27% (10n) require immediate intervention. In Ciudad Chihuahua in the Hospital General Regional No. 1, the care of this group of patients started in August 2018. To date, more than 800 consultations have been provided, of which 427 patients have been diagnosed with congenital heart disease and 35.8% (153n) of these patients have been considered of priority care for complementary diagnosis or surgery. There are 141 patients on the waiting list for surgery, which are 33% of this population, from which 88% (124n) require first-time surgery and 12% (17n) require a reintervention.

Ciudad Juárez is the most economically important city and the most populated in the state and has one of the highest infant mortality rates in the country, justifying the need for a third level of medical care.

It is estimated that there are just over 220 cases waiting between Ciudad Juárez and Chihuahua with congenital heart disease alone, not contemplating the adult population that requires surgery. Evaluating the costs of care for hospitalized patients waiting for definite management or attending with some complication, it is estimated that there is a reduction in care costs of approximately 46% for one case in the Hospital Infantil de Especialidades and a 65% in the case of the IMSS, in addition to the reduction in morbidity and mortality. The association can support with resources for diagnostic or therapeutic procedures by cardiac catheterization and in the acquisition of specific supplies by surgery, especially some type of biological or synthetic grafts that are usually used in cardiac surgery.

Our History

This foundation is a newly established non-profit, non-governmental organization, all members are doctors related to the area of pediatric cardiology that have been working in the area for at least 6 years in Ciudad Juárez and about 2 years in Ciudad Chihuahua and have collected information that was exposed previously. This information, aside from exposing the need for a third level, also poses what the institutions can save to generate better care in all the services. The physiopathology of the congenital heart disease points out specific moments of attention, that in this case is relevant in early stages of life, reflecting a high mortality of 20% compared to countries with established care protocols with mortalities no greater than 3% even in the most complex cases. In the adult population, it impacts mainly on patients in the stage of productive life that institutions cost in disabilities, disability pensions, demands, and frequent hospital care.

Mission

This is an organization based on Cd. Juárez that has been created with the purpose of supporting the vulnerable population with a heart disease diagnosis that requires medical and/or surgical care in view of a saturated health system that depends on other cities to solve its health problems with an important impact in the life of this group of patients.

To generate a close relationship with the health system to manage the efficiency in the medical and surgical care with the support of the Foundation to promote the third level of medical care in Cd. Juárez and the state of Chihuahua.
